Please submit the following to ksykes@littlerock.gov in order for your application to be complete. 

  1. Current Business License
  2. Valid Arkansas Health Department License
  3. Liability Insurance at a coverage of a minimum of one million dollars ($1,000,000) with the City of Little Rock listed as an additionally insured party
  4. Picture of your food truck/booth. 

 

By submitting this form I am acknowledging that I have correctly filled out the above form and that I have read and will abide by the guidelines. I also agree to assume full responsibility for my conduct and for all of my property I bring on the premises.  I will not hold the City of Little Rock responsible for any damages or personal injury or theft or any act that may occur at Music in the Park.  I understand that I am responsible for paying all sales taxes. I understand that my name or business name and any photos may be used for promotional and/or informational purposes.  I acknowledge I am self-contained and require no electricity, water, or other service from the Parks Department. I understand that once accepted, all fees are non-refundable unless the event is cancelled due to inclement weather.  Then, I will receive 50% of my application fee back.
